Dear Javier,

Please, find attached the Official Document referring of the
person entitled to sing for FUSP, that is José Roberto Drugowich de Felicio.

Could you please change the name inform C to José Roberto
Drugowich de Felicio in order to FUSP send the document.

Dear all,

Just for clarify that if the authorised person to sign the Financial
Statement has changed, the P.O. wants an empower letter stated that the
person who signed the Form C (at the place of the previous person) is
entitled to sign for the company. Please provide an original signed letter.

Dear Anna, all,

Sorry if there is a misunderstanding.

I meant to print and sign only the pages of your company.

I'll receive them from each partner and I'll send all together to the P.O.
as a complete set.

Don't worry if you cannot stamp the document. As far as I know it is not
mandatory but it would be advisable.

Hi Chantal, all

The P.O. has requested original signed copies. It is mandatory.

Please print your copies (2 of the FORM-C-3, 2 of the adjustment to P1 + 2
of the adjustment to P2), stamp and sign (handwriting) them.

It is an explicit request from the Commission.

Don’t forget to include the CFS.

Dear all,

This e-mail is to inform you that yesterday the Commission closed the NEF
session, so the process of the final payment process has started.

We have 15 days to send to the Commission the original signed copies of the
FORM-C, adjustments and CFS.

Deadline: December 18th, 2015

If we don’t reach this deadline, the Commission will stop the clock of the
payment process.

So, I kindly ask you to attend this important e-mail urgently.

Please find attached the documents to be printed, stamped and signed (please
use a blue pen). Don’t forget to include the CFS if needed.

Please send two original copies to the following address by courier:
