[Fiware-lab-help] Problem with community account

Dear,

I have tried to upgrade to community account as advised in the email I 
have received, and also was following steps described in the link given 
in email.

However, I cannot upgrade account for some reason. Whenever I fill the 
form and press submit button it is not accepted. Please find below 
screenshot of the upgrade form.
