I suggest that you use as a model  the one that we created for that purpose. For general guidance use the guidelines that we provided here:

More precisely,  here you have the concrete guide for the Unit Testing Plan & report:

I have already provided Pascal my feedback to the SSS Unit Testing Plan and there are a few things to modify . Please take the above link as the reference, it also points to a very good example.
