[ICT-13] Partners information form for Section 4 of the proposal

Hi all,

   We want to start collection of the forms with the description of 
partners involved in the project.

   You can find here

the template that you should use.  Besides, you can find the form 
corresponding to the FIWARE Foundation

as an example.

   Please respect the guidelines and formatting of the several sections.

   Deadline for delivering the forms is this Monday EOD.
