Hi Dragomir,
I am not sure i get the problem you are having. As described in the guide (
http://forge.fiware.org/plugins/mediawiki/wiki/fiware/index.php/FIWARE_Lab:_Upgrade_to_Community_Account#How_can_i_request_for_a_Community_Account_upgrade),
the account request is created here:
http://help.lab.fi-ware.org clicking on "Request Community Account Upgrade".

I tested and it works, I get an email with the information that a ticket
with my request for account upgrade was created.

the thing works as follow:
- you submit a form;
- a ticket is created;
- someone (from the accelerator you target or from FIWARE Lab) follows up
with you via email;
- if your request is approved (because they verified you part of an
accelerator or your work as a value for FIWARE), you get upgraded and
notified via email.
