[Fiware-membership-application] Individual membership request

Hello,

I would like to join the FIWARE foundation as an individual member. We 
have participated to a FIWARE accelerator as an SME, and work and 
develop with FIWARE technologies in Elmitel.

Could you please let me know which information is required, and send me 
the necessary documentation?
