Harmonized the introductory description with NGSI 10 binding
Made support of interaction with /contextEntities/{EntityID}/attributes and /contextEntityTypes/{typeName}/attributes optional.

Moved the contents of chapter 5 into the beginning of chapter 4.


This is now also ready for publication from my point of view, but please check.
